Then, either one four-part DD Form 1348 (MVO)
or one NAVSUP Form 1250-1 (MVO) will be
prepared for each material category. Each DD
Form 1348 and NAVSUP Form 1250-1 will
include the following data entries (see figs. 5-4 and
5-5 for examples):
