Transportation
for Dependents,
DD Form 884 (fig.
11-6).
Information
required
on
this
form
is
assistance
to the transferring
member,
if he or she
requests it. This form must be completed for overseas
and CONUS travel
when the member is under PCS
orders.

The NAVPTO
confirms
travel
arrangements
by
issuing a port call when the following
documents have
self-explanatory.
