using a DD Form 1155. The DD Form 250 is a
multipurpose document that serves principally to
provide evidence of inspection or acceptance at
either the material source or its destination and
to substantiate contract payments. When material
is furnished directly to your ship by a commercial
contractor, and inspection or acceptance at
destination is indicated on the DD Form 250, the
material should be inspected by the appropriate
technical personnel and completed by that
representative and the receiving SK.
Upon receipt of the material, the SK should
make sure the received quantity is the same as the
ordered quantity. If the quantities are the same,
he or she should put a check mark in block 17. If
the quantities are different, the actual quantity re-
ceived should be entered and circled below the
quantity shipped. The quantity damaged, short, or
unacceptable may be shown and an explanation
as to the reason should be made in writing on the
form by the SK receiving the material. (See fig.
