PS Form 7380 in duplicate. Submit the original to the
Material Distribution Center, Topeka, KS, and retain
the duplicate copy in the post office files.
Requisitions are numbered consecutively
beginning with number 1 on 1 October each year.
Requisitions must be reviewed and signed by the MPO
supervisor or the COPE. Complete PS Form 7380 as
follows (see figure 12-8):
Block 1: Type MPO supervisor or COPE along with the
complete mailing address of your MPO.
